We can issue a replacement pass for an admin fee of £5 – we’ll just need one of the following bits of information
- Proof of original purchase – a receipt or copy of a bank statement, online booking confirmation, OR:
- Proof of address – Something like a recent utility bill
What do I need to do?
- Download a Lost or Stolen Annual Pass Form
- Complete the form, attach your proof of purchase/address and email it back to us or post it to:
Annual Pass Replacement, Eureka! The National Children’s Museum, Discovery Road, Halifax, HX1 2NE
Please allow 5 working days for us to process your replacement and post it back to you or, if you prefer, you can arrange to collect it when you visit.
We’re not able to issue replacement annual passes at our admissions tills or information desk.
